Critical Illness Insurance
Critical Illness Insurance Provides a Benefit Payment if You or a Covered Family Member are Diagnosed with a Covered Illness.
You don't have to enroll for MDLZ-sponsored medical coverage to elect Critical Illness Insurance. There are two coverage levels offered through Voya Employee Benefits (Voya):
- $15,000
- $30,000
The benefit payment is based on the coverage level you elect for yourself ($15,000 or $30,000), and the percentage that’s payable for the covered illness (i.e., cancer, heart attack, stroke). Coverage is also available for your spouse/domestic partner and/or child(ren) at 100% of the coverage level you elect for yourself. Your cost—which you pay for on an after-tax basis through payroll deductions—depends on your age, coverage level, and who you cover.
You can use the benefit payment to help pay for any type of bill or expense, including:
- Your mortgage or rental payments
- Travel expenses if you’re receiving care in a different city
- Living expenses such as bills, electricity, and gas
- Lost income due to missed time from work
A Wellness Benefit is included in your Critical Illness Insurance coverage. With the Wellness Benefit, you receive a $50 annual benefit payment if you complete an eligible health screening test on or after your coverage effective date—regardless of whether there’s any out-of-pocket cost to you. You may also receive a benefit payment if your covered spouse and/or child completes a health screening test on or after your coverage effective date.
