Any change to coverage must be consistent with your qualified life event.<\/p>\n\t<h3>Examples of qualified life events<\/h3>\n<p>Examples of qualified life events include:<\/p>\n<ul>\n<li>Birth<\/li>\n<li>Adoption<\/li>\n<li>Marriage<\/li>\n<li>Divorce<\/li>\n<li>Death of a spouse<\/li>\n<li>Change in employment status<\/li>\n<li>Change in residence that results in a gain or loss of eligibility for coverage<\/li>\n<\/ul>\n<p>For a complete list of qualified life events, access <a href=\"https:\/\/mdlzmybenefitsonline.com\" target=\"_blank\" rel=\"noopener\">MyBenefits Online<\/a>.<\/p>\n\t<h3>MAKING CHANGES TO YOUR COVERAGES<\/h3>\n<p>Within 30 days of the date of the qualified life event, contact the Mondel\u0113z International Benefits Center at <a href=\"tel:18008878807\">1-800-887-8807<\/a>\u00a0(Option 1 &#8211; Benefits; Option 1 &#8211; Health &amp; Welfare Benefits). Representatives are available Monday through Friday, 7:00 a.m. to 6:00 p.m. (Central Time).
