Coverage is available for your spouse\/domestic partner ($5,000 increments, up to $250,000) and\/or child(ren) equal to $10,000. You can review the coverage options available and your cost for each option on <a href=\"https:\/\/mdlzmybenefitsonline.com\/\" target=\"_blank\" rel=\"noopener\">MyBenefits Online<\/a>.<ul><li>If you\u2019re a new hire, you have a one-time opportunity to elect the following coverages without having to provide Evidence of Insurability (EOI):<ul><li><strong>For yourself:<\/strong> Up to $250,000<\/li><li><strong>For your spouse\/domestic partner:<\/strong> Up to $50,000<\/li><li><strong>For your child(ren):<\/strong> $10,000 (EOI not applicable)<\/li><\/ul><\/li><li>If you want to elect or make changes, you can do so during a future Annual Enrollment and\/or during the year if you experience a qualified life event (EOI may be required).<\/li><\/ul><\/li><li><span class=\"green-text\"><strong>VADD Insurance:<\/strong><\/span> You can purchase coverage for yourself should you be injured or you die in an accident. Your coverage equals the amount of Optional Life Insurance coverage you elect. So, if you elect Optional Life Insurance coverage for yourself equal to $300,000 and you elect $75,000 of Optional Life Insurance coverage for your spouse, your VADD Insurance coverage equals $375,000. You can also elect VADD coverage for your eligible dependents. See <a href=\"https:\/\/mdlzmybenefitsonline.com\/\" target=\"_blank\" rel=\"noopener\">MyBenefits Online<\/a> for the coverage options available and each option\u2019s cost.<\/li><li><span class=\"green-text\"><strong>LTD Coverage:<\/strong><\/span> 60% of your annual base pay if your disability extends beyond the initial 26 weeks of STD. MDLZ will pay the cost of this coverage, with the LTD benefit then being taxable to you.
