As health care costs continue to rise, the Company needs to take additional steps for 2026 to ensure that the Mondelēz Global Pre-65 Retiree and Long-Term Disability Medical Plan (the Pre-65 Plan) remains sustainable. As a result, the Company will share a portion of these higher costs through certain Pre-65 Plan changes for next year.

• Increased Medical Deductibles: Deductibles will increase under two of the Pre-65 Plan’s coverage options. In addition, the names of these options will change as follows:
  • $500 Deductible option → $750 Deductible option
  • $2,000 Deductible option → $2,250 Deductible option

• So, for in-network services, the individual deductibles for Employee Only coverage will increase by $250 for each of these options. If you cover any other family members and receive in-network services, the family deductible for these coverage options will increase by $500. Deductible amounts under the $3,500 Deductible coverage option will remain the same for 2026.

• Increased Out-of-Pocket Maximums: Similar to the above, the Company will share the impact of higher health care costs by increasing out-of-pocket maximums for the same two coverage options (so for in-network services a $250 increase to the individual out-of-pocket maximum for Employee Only coverage; and a $500 increase to the family out-of-pocket maximum if you cover any other family members). Out-of-pocket maximums under the $3,500 Deductible coverage option will remain the same for 2026.

• Changes to Prescription Drug (Rx) Coverage:
  • If You Elect the $750 Deductible Option for 2026: You’ll need to meet your deductible before the Pre-65 Plan pays benefits for covered prescriptions—just like you do for other covered medical services. This change will align with how Rx coverage works across all three of the Pre-65 Plan’s options.
  • Increased Copays and Min/Max Coinsurance Limits: These Rx provisions have remained the same since 2018. For 2026, however, copays and min/max coinsurance limits will increase under all three medical coverage options.

• GLP-1s No Longer Covered as Preventive: GLP-1s will still be covered by the Pre-65 Plan. But, with this change, GLP-1s will be charged like other medications that are used to treat medical conditions.
• Remember: If you’re prescribed an anti-obesity medication (AOM), including GLP-1s for weight-loss management, you’ll need to enroll in and engage with Vida Health before your AOM (or GLP-1) will be eligible for prescription drug benefits.
