This amount is &#8220;imputed income.&#8221;<\/p>\n<p><em>*If you meet the eligibility requirements for coverage as a pre-65 retiree; or you&#8217;re a former employee who&#8217;s considered disabled for purposes of the Mondel\u0113z Global Long-Term Disability Plan (LTD Plan); you may be eligible for medical coverage under the Mondel\u0113z Global Pre-65 Retiree and Long-Term Disability Medical Plan (the Pre-65 Plan). See the &#8220;Your Guide to the Mondel\u0113z Global LLC Retiree Health and Life Benefits Plan&#8221; for details regarding eligibility requirements (you can access this document in the Reference Center on <a href=\"http:\/\/www.mdlzmybenefitsonline.com\/\" target=\"_blank\" rel=\"noopener\"><em>MyBenefits Online<\/em><\/a>). Representatives are available Monday through Friday, 7:00 a.m. to 6:00 p.m. (Central Time).<\/li><\/ul><h4 class=\"red-text\"><strong>Medicare-eligible at age 65<\/strong><\/h4><ul><li>Eligibility under the Pre-65 Plan ends once an individual becomes eligible for Medicare at age 65.<\/li><li>Medicare-eligible individuals can choose coverage from a variety of medical and prescription drug plans through the Post-65 Via Benefits Exchange\u2014our partner in offering access to these coverages. The Post-65 Via Benefits Exchange can provide more information and confirm whether you\u2019re eligible for Company funding toward your post-65 health care costs through a retiree Health Reimbursement Arrangement.<\/li><li>For assistance, access the<a href=\"https:\/\/my.viabenefits.com\/mondelez\" target=\"_blank\" rel=\"noopener\"> Via Benefits website<\/a> or contact the Post-65 Via Benefits Exchange at <strong><a href=\"tel:18662010758\">1-866-201-0758<\/a><\/strong>. As a result, you have the opportunity to review and make changes to your current coverages during the Annual Enrollment period. To confirm your eligibility, contact the Mondel\u0113z International Benefits Center at <a href=\"tel:18008878807\">1-800-887-8807<\/a> (Option 1 \u2013 Benefits; Option 1 \u2013 Health &amp; Welfare Benefits).<\/p><p>As an eligible disabled former employee, MDLZ provides your coverage under the Pre-65 Plan\u2014regardless of your age and service on the day your employment terminates. As long as you\u2019re considered disabled for LTD Plan purposes:<\/p><ul><li>You remain eligible for coverage under the Pre-65 Plan\u2014even if you\u2019re older than age 65 and Medicare-eligible.<\/li><li>Your eligible spouse\/domestic partner (if applicable), as well as your otherwise eligible dependents, continue to be eligible for coverage under the Pre-65 Plan\u2014even if they\u2019re older than age 65 and Medicare-eligible. And, when your eligibility ends (i.e., if your LTD Plan benefits end at age 65), your dependents\u2019 eligibility ends at the same time.<\/li><li>MDLZ provides you with a subsidy, which you can use toward the cost of your coverage. Your remaining cost depends on the coverage option you elect and who you enroll for coverage. See the \"Your Guide to the Mondel\u0113z Global LLC Retiree Health and Life Benefits Plan\" and the SPDs and other documents for specific eligibility requirements (access these documents in the <em>Reference Center<\/em> on <a href=\"https:\/\/mondelez.ehr.com\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">MyBenefits Online<\/a>).<\/p><p>Proof of eligibility, including a Social Security Number if you\u2019re enrolling a dependent via <a href=\"https:\/\/mondelez.ehr.com\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">MyBenefits<\/a><a href=\"https:\/\/mondelez.ehr.com\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\"> Online<\/a>, will be required within 90 days after enrollment. If your dependent doesn't have a Social Security Number, you'll need to call the Mondel\u0113z International Benefits Center at <a href=\"tel:18008878807\">1-800-887-8807<\/a> (Option 1 - Benefits; Option 1 - Health &amp; Welfare Benefits) to add your dependent.<\/p><p>Any misrepresentations or inaccurate information you provide could result in loss of coverage.
