As a Part-Time Sales Service Representative (PT SSR), you’re considered “newly eligible” for certain MDLZ-sponsored benefits and coverages if:
- During your first 12 months of employment you reach a 30-hour a week average service requirement. If you do, you can enroll for certain coverages for the next 12-month period (coverages you elect begin on your first service anniversary date and continue until your second anniversary date, with the option to change your elections during Annual Enrollment).
- You reach a 30-hour a week average service requirement over a 12-month period pre-determined by the Company*. If you do, during the Annual Enrollment period you can enroll for certain coverages for the next calendar year (elected coverages take effect on January 1 and continue through December 31).
If you're a newly eligible PT SSR:
- Make your health and welfare coverage elections on MyBenefits Online within 30 days of your initial eligibility date. If you don't elect coverage during your initial 30-day enrollment period, your earliest opportunity to enroll may not be until Annual Enrollment.
- Make your 401(k) Savings Plan elections within 30 days of your eligibility date. By doing so, your elective contributions begin approximately the first pay cycle 30 days after your eligibility date.
*Each year, the Company measures which PT SSRs have met—on average—a 30-hour a week average service requirement during a specified measurement period. The specified measurement period is a 12-month look-back period that generally takes place from October to October. If it’s determined that you’ve met the 30-hour a week average service requirement during the look-back measurement period, you receive Annual Enrollment materials and can elect the MDLZ-sponsored benefits and coverages for which you’re eligible for the following calendar year.
You have 30 days to enroll
Make your health and welfare coverage elections within 30 days of your eligibility date (your eligibility date is your newly eligible date), or during the Annual Enrollment period. If you don’t enroll by the deadline, you won’t have medical coverage, a Health Savings Account (HSA), or a Health Care Flexible Spending Account (Health Care FSA) for the current year or the next calendar year (if applicable)—unless you experience a qualified life event.
Make your 401(k) Savings Plan elections within 30 days of your eligibility date. By doing so, your elective contributions begin approximately the first pay cycle 30 days after your eligibility date.

- Read the benefits information on this Benefits Made Right site and explore the coverage options available—categorized as HEALTH, MONEY, MORE, and THE RIGHT YOU.
- Enroll in your health and welfare coverages and use the decision-support tools available on MyBenefits Online. For personal assistance or to enroll by phone, contact the Mondelēz International Benefits Center at 1-800-887-8807 (Option 1 – Benefits; Option 1 – Health & Welfare Benefits). Representatives are available Monday through Friday, 7:00 a.m. to 6:00 p.m. (Central Time).
- Make your 401(k) Savings Plan elections through Fidelity Investments® (Fidelity services our 401(k) Savings Plan.) For personal assistance or to reach Fidelity via the Mondelēz International Retirement and Savings Plan Center by phone, call 1-866-612-4582. Representatives are available Monday through Friday, 7:30 a.m. to 7:30 p.m. (Central Time).

Need help deciding what will best support The Right You?
Use the online tools and resources available for your health and welfare coverages on MyBenefits Online, and then use MyBenefits Online to enroll.
Key points to remember
- Be prepared if enrolling dependents. Have Social Security Numbers, dates of birth, and proof of eligibility ready (see eligibility rules).
- Designate your beneficiaries (as applicable) through:
- MyBenefits Online for Business Travel Accident (BTA) Insurance; and
- The NetBenefits website for the HSA and 401(k) Savings Plan.Note: If you're married, your spouse is automatically your 401(k) Savings Plan beneficiary (unless you elect otherwise).
- Decide if a tax-savings account makes sense and best supports The Right You. If so, make an HSA or Health Care FSA contribution election. Even though you make a contribution election on MyBenefits Online, the tax-savings accounts are serviced by Fidelity. Once your account is established via MyBenefits Online, you receive additional information directly from Fidelity (including how to activate your account online, request additional debit cards, invest your HSA contributions, and elect beneficiaries for your HSA).
- Understand enrollment rules, including what happens if you don’t enroll.
- Be sure to review and submit your health and welfare coverage elections on MyBenefits Online. Shortly after you enroll, you'll be able to view your online Benefit Summary. Review your coverage elections carefully. If there's an error, promptly contact the Mondelēz International Benefits Center at 1-800-887-8807 (Option 1 – Benefits; Option 1 – Health & Welfare Benefits) so that it can be corrected as soon as possible.
- Review your 401(k) Savings Plan Welcome Packet that you receive at your home approximately 10 days after your eligibility date. Consider your contribution and investment options, and submit your elections through the NetBenefits website. Then, watch your email or mailbox for your 401(k) Confirmation Statement.
