{"id":281,"date":"2024-06-05T13:42:39","date_gmt":"2024-06-05T13:42:39","guid":{"rendered":"https:\/\/mdlzbmr.com\/ptssr\/?page_id=281"},"modified":"2026-02-26T23:31:54","modified_gmt":"2026-02-26T23:31:54","slug":"eligibility-rules","status":"publish","type":"page","link":"https:\/\/mdlzbmr.com\/ptssr\/eligibility-rules\/","title":{"rendered":"Eligibility Rules"},"content":{"rendered":"\n\n\t\t<h2>Eligibility Rules<\/h2>\t\n<h2>\n\t\t\tmdlz offers certain health and welfare coverages to those who meet the hours-service requirement*\t<\/h2>\n\t<p>As an eligible Part-Time Sales Service Representative (PT SSR), you may enroll the following eligible dependents for medical coverage:<\/p>\n<ul>\n<li><strong>Your legal spouse<\/strong><\/li>\n<li><strong>Your same- or opposite-sex domestic partner<\/strong><\/li>\n<li><strong>Your children<\/strong><\/li>\n<\/ul>\n<p>NOTE: If your domestic partner or domestic partner&#8217;s child isn&#8217;t your tax dependent, the full cost of their medical coverage is taxable income to you. This amount is &#8220;imputed income&#8221; and will be included in the income amount shown on your pay statement and W-2 form. Applicable taxes on imputed income will be withheld each pay period.<\/p>\n<p><em>*An hours-service requirement (i.e., average hours worked during a specified measurement period) applies for PT SSRs. See <a href=\"https:\/\/mdlzbmr.com\/ptssr\/new-hires\/\"><em>Newly Eligible<\/em><\/a> for more details. For more information regarding the specific coverage options for which you&#8217;re eligible and\/or enrolled in during the year, go to <\/em><a href=\"http:\/\/www.mdlzmybenefitsonline.com\/\" target=\"_blank\" rel=\"noopener\"><em>MyBenefits Online<\/em><\/a><em>.<\/em><\/p>\n<p>&nbsp;<\/p>\n\t<style>#sp-ea-691 .spcollapsing { height: 0; overflow: hidden; transition-property: height;transition-duration: 300ms;}#sp-ea-691.sp-easy-accordion>.sp-ea-single {margin-bottom: 10px; border: 1px solid #e2e2e2; }#sp-ea-691.sp-easy-accordion>.sp-ea-single>.ea-header a {color: #ffffff;}#sp-ea-691.sp-easy-accordion>.sp-ea-single>.sp-collapse>.ea-body {background: #fff; color: #444;}#sp-ea-691.sp-easy-accordion>.sp-ea-single {background: #eee;}#sp-ea-691.sp-easy-accordion>.sp-ea-single>.ea-header a .ea-expand-icon { float: left; color: #ffffff;font-size: 16px;}<\/style><div id=\"sp_easy_accordion-1720715321\"><div id=\"sp-ea-691\" class=\"sp-ea-one sp-easy-accordion\" data-ea-active=\"ea-click\" data-ea-mode=\"vertical\" data-preloader=\"\" data-scroll-active-item=\"\" data-offset-to-scroll=\"0\"><div class=\"ea-card sp-ea-single\"><h3 class=\"ea-header\"><a class=\"collapsed\" id=\"ea-header-6910\" role=\"button\" data-sptoggle=\"spcollapse\" data-sptarget=\"#collapse6910\" aria-controls=\"collapse6910\" href=\"#\" aria-expanded=\"false\" tabindex=\"0\"><i aria-hidden=\"true\" role=\"presentation\" class=\"ea-expand-icon eap-icon-ea-expand-plus\"><\/i> ELIGIBLE CHILDREN<\/a><\/h3><div class=\"sp-collapse spcollapse spcollapse\" id=\"collapse6910\" role=\"region\" aria-labelledby=\"ea-header-6910\"> <div class=\"ea-body\"><p>Your child is eligible for coverage, up to age 26, provided he or she is your (or your spouse's or domestic partner's):<\/p><ul class=\"reduce-top-margin\"><li>Biological child<\/li><li>Stepchild<\/li><li>Foster child<\/li><li>Legally adopted child, or child placed in your and your spouse's (or your and your domestic partner's) home for legal adoption<\/li><li>Child for whom adoption procedures have started<\/li><li>Child for whom a court has appointed you (or your spouse or domestic partner) as the legal guardian<\/li><li>Child for whom you (or your spouse or domestic partner) have been ordered to cover through a Qualified Medical Child Support Order (QMCSO)<\/li><\/ul><p>To be eligible for coverage, your spouse's or domestic partner\u2019s child also must be primarily dependent on you (or your spouse or domestic partner) for more than one-half of his or her support and live with you (or your spouse or domestic partner) for more than one-half of the calendar year.<\/p><\/div><\/div><\/div><div class=\"ea-card sp-ea-single\"><h3 class=\"ea-header\"><a class=\"collapsed\" id=\"ea-header-6911\" role=\"button\" data-sptoggle=\"spcollapse\" data-sptarget=\"#collapse6911\" aria-controls=\"collapse6911\" href=\"#\" aria-expanded=\"false\" tabindex=\"0\"><i aria-hidden=\"true\" role=\"presentation\" class=\"ea-expand-icon eap-icon-ea-expand-plus\"><\/i> VERIFY YOUR DEPENDENTS<\/a><\/h3><div class=\"sp-collapse spcollapse spcollapse\" id=\"collapse6911\" role=\"region\" aria-labelledby=\"ea-header-6911\"> <div class=\"ea-body\"><p class=\"reduce-top-margin\">You're responsible for ensuring that each individual you enroll for coverage under the Mondel\u0113z Global Medical Plan (Medical Plan) meets the Medical Plan's eligibility requirements at all times. Refer to \u201dThe Mondel\u0113z Global LLC Group Benefits Plan\u201d and the SPDs for specific eligibility requirements (access these in the <em>Reference Center<\/em> on <span style=\"text-decoration: underline\"><a href=\"https:\/\/mondelez.ehr.com\/\" target=\"_blank\" rel=\"noopener\">MyBenefits Online<\/a><\/span>).<\/p><p>Proof of eligibility, including a Social Security Number if you\u2019re enrolling your dependent via\u00a0<a href=\"https:\/\/mondelez.ehr.com\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">MyBenefits<\/a><a href=\"https:\/\/mondelez.ehr.com\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\"> Online<\/a>, will be required within 90 days after enrollment. If your dependent doesn\u2019t have a Social Security Number, you\u2019ll need to call the Mondel\u0113z International Benefits Center at\u00a0<strong><a href=\"tel:18008878807\">1-800-887-8807<\/a><\/strong>\u00a0(Option 1 \u2013 Benefits; Option 1 \u2013 Health &amp; Welfare Benefits) to add your dependent.<\/p><p>Any misrepresentations or inaccurate information you provide could result in loss of coverage.
