MyBenefits Online provides access to your personal health and welfare coverage information, carrier/benefit vendor websites, and phone numbers. It also provides access to detailed benefit summaries, comparisons, and legally required documents—such as the “Your Guide to the Mondelēz Global LLC Group benefits Plan,” SPDs,* SMMs to those SPDs, and Summaries of Benefits and Coverage (SBCs). These are all posted in the Reference Center on MyBenefits Online. Paper versions of these documents also are available, free of charge, by contacting the Mondelēz International Benefits Center at 1-800-887-8807 (Option – Benefits; Option 1 – Health & Welfare Benefits).

*Certain SPDs are updated annually. If you have any questions regarding a specific coverage, please reach out to the individual carrier directly.
