The Mondelēz Global Vision Plan (Vision Plan) provides vision coverage through EyeMed.

In-network features include:

Access to a broad network of EyeMed providers.
Pay nothing for a routine comprehensive eye exam, limited to one per calendar year.
Pay a $15 copay for eyeglass lenses (single vision or bifocal lenses).
Use allowance to help cover frames ($150 per year) or contact lenses ($140 per year).
Receive discounts for other vision materials.

Keep in mind, it may cost you more if you receive services from an out-of-network provider. Note: You don't have to enroll for MDLZ-sponsored medical coverage to elect vision coverage.
