If you\u2019d rather pay more in medical contributions to keep your potential cost-sharing amounts lower if\/when you need medical care, consider the $750 Deductible option.<\/li><li><span class=\"blue-text\"><strong>Where you get care:<\/strong><\/span> You can pay less in contributions from your pay if you select a coverage option with a smaller group of providers called a\u00a0<em>select network<\/em>\u00a0(if offered by a carrier in your area). If you prefer a larger selection of providers, consider an option with\u00a0<em>a broad network<\/em>\u00a0instead. In either case, it\u2019s always most cost effective to receive care from an in-network provider. Confirm your provider\u2019s network participation by using the provider look-up tool on\u00a0<a href=\"https:\/\/mondelez.ehr.com\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">MyBenefits Online<\/a>\u2015especially since network participation can change over time. It\u2019s also good to confirm network participation directly with your provider before you receive services.<\/li><li><span class=\"blue-text\"><strong>Special coverage needs:<\/strong><\/span>\u00a0Consider the specific care you or your family members may need (for example, regular check-ups with a specialty provider). To understand how the carriers will cover your care, use their cost look-up tools on\u00a0<a href=\"https:\/\/mondelez.ehr.com\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\">MyBenefits Online<\/a>.<\/li><li><span class=\"blue-text\"><strong>Processes and standards vary by carrier:<\/strong><\/span>\u00a0Two carriers may treat a covered service differently when making a benefit determination. As a result, how services are covered and what you may pay can vary across carriers. Also, carriers might adjust costs for certain services (for example, negotiated rates for certain covered services and telemedicine costs) from year to year. If you have questions about how a specific service is covered, contact the carrier directly.<\/li><\/ul><h4 class=\"blue-text\"><strong>Dental and vision<\/strong><\/h4><p>Keep in mind:<\/p><ul><li><span class=\"blue-text\"><strong>You have two dental coverage options:<\/strong><\/span>\u00a0In addition to offering coverage for preventive, basic, and major dental care services, one of the options offers coverage for orthodontia services. Consider which will best meet your needs.<\/li><li><span class=\"blue-text\"><strong>If you or your family members need vision care:<\/strong><\/span>\u00a0Vision coverage for exams and eyewear may be right for you.<\/li><\/ul><\/div><\/div><\/div><div class=\"ea-card sp-ea-single\"><h3 class=\"ea-header\"><a class=\"collapsed\" id=\"ea-header-3091\" role=\"button\" data-sptoggle=\"spcollapse\" data-sptarget=\"#collapse3091\" aria-controls=\"collapse3091\" href=\"#\" aria-expanded=\"false\" tabindex=\"0\"><i aria-hidden=\"true\" role=\"presentation\" class=\"ea-expand-icon eap-icon-ea-expand-plus\"><\/i> Money<\/a><\/h3><div class=\"sp-collapse spcollapse spcollapse\" id=\"collapse3091\" role=\"region\" aria-labelledby=\"ea-header-3091\"> <div class=\"ea-body\"><p>Key points to keep in mind:<\/p><ul><li><span class=\"green-text\"><strong>Maximize your health care savings potential (if eligible):<\/strong><\/span> Take advantage of the Health Savings Account\u2019s (HSA\u2019s) triple-tax advantage\u2014contribute (and build upon HSA matching contributions from the Company if eligible); pay for eligible medical, prescription drug, dental, and vision expenses; and earn interest\/investment returns\u2014all tax-free. Remember, an HSA is paired with the Medical Plan\u2019s $2,250 and $3,500 Deductible options.<\/li><li><span class=\"green-text\"><strong>Think through the potential tax savings of contributing to the Flexible Spending Accounts (FSAs):<\/strong><\/span> If you\u2019re a new hire, note that any amount you may have contributed to an FSA since January 1, 2026 (prior to your employment with MDLZ ) will count toward the annual contribution limits that apply to the FSAs. If you decide to participate in an FSA, plan carefully. By law, you'll forfeit any amount for the year left in your FSA after the claims submission window ends on March 31, 2027.<\/li><li><span class=\"green-text\"><strong>Consider whether additional disability, life, and accident protection makes sense for you:<\/strong><\/span> Understand what coverages you automatically receive from MDLZ, and the options you have to purchase additional coverages. If you\u2019re a new hire, you have a one-time opportunity to elect certain Optional Life Insurance coverage amounts without having to provide Evidence of Insurability (EOI). When you first enroll and during Annual Enrollment, be sure you review your beneficiaries and update them if needed.<\/li><li><span class=\"green-text bold\">Don't wait to start thinking about your financial well-being and future financial security:<\/span>\u00a0The 401(k) Savings Plan can help you meet your goals. If you\u2019re a new hire, automatic enrollment applies at the time you\u2019re first eligible, unless you actively make a different election.
