- From MyBenefits Online:<\/strong> Once you’re logged in, select Access the App <\/em>from the MyChoice Mobile App tile on the home page. Then, scan the QR code (this code is unique to you for downloading purposes so don’t share).<\/li>\n

When deciding whether to enroll in coverage under the Mondel\u0113z Global Medical Plan (Medical Plan), provided you’re eligible given your negotiated CBA, remember:<\/p>\n

- The Medical Plan coordinates benefits with other plans<\/strong>\u00a0that may cover you and\/or your covered family members. This helps prevent duplication of benefits for the same services. So, if you elect employee plus one or more dependents under the Medical Plan and your spouse or domestic partner elect family coverage under his or her employer’s plan, the Medical Plan generally coordinates benefits as follows:\n

- The Medical Plan is “primary” for you.<\/li>\n
- The Medical Plan is “secondary” for your spouse or domestic partner.<\/li>\n
- If you and your spouse cover your dependent children under both of your plans, primary coverage for the children will generally be provided by the plan of the parent with the birth date earlier in the year.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\t
401(k) Savings Plan<\/strong><\/h4>\n
If your union’s negotiated CBA allows you to participate in the 401(k) Savings Plan, you can make your elections:<\/p>\n

\n\t\t\tWhen you can change your elections\t<\/h2>\n\t
The following rules apply:<\/h4>\n

- The coverage elections you make generally continue through December 31 of each year.<\/strong>\u00a0During the year, you can change most coverage elections\u00a0only<\/strong>\u00a0if you experience a qualified life event. See\u00a0MyBenefits Online<\/a><\/strong> for details regarding qualified life events.<\/li>\n
- If you’re eligible for Long-Term Disability (LTD) coverage<\/strong>, you may elect or change your LTD coverage at any time throughout the year (including dropping coverage). But, to avoid having to provide Evidence of Insurability (EOI), you must enroll within your original 30-day enrollment window.<\/li>\n
- If you’re eligible for Optional Life or Supplemental Life Insurance coverages<\/strong>, you may elect or change your coverage if you experience a qualified life event (or during Annual Enrollment), though EOI may be required.<\/li>\n
- If you’re eligible to participate in the 401(k) Savings Plan<\/strong>, you may change your contribution and investment elections at any time. Log on to the NetBenefits website<\/a> to make changes to your contribution rate, or to change your current and future investment elections.<\/li>\n<\/ul>\n\t\t\t\t

If you don’t enroll by the deadline, you won’t have certain MDLZ coverages for which you’re eligible (as determined by your negotiated CBA) for the current year. Depending on your CBA, this may mean you won’t have medical, dental, vision, a Health Care Flexible Spending Account (Health Care FSA), a Dependent Day Care FSA, and\/or other voluntary coverages for the current year. Your next opportunity to enroll will be Annual Enrollment (for the next calendar year)-unless you experience a qualified life event, in which case you can enroll mid-year at the time of your life event.
