The Mondelēz Global Vision Plan (Vision Plan) provides vision coverage through EyeMed.

In-network features include:

  • Access to a broad network of EyeMed providers.
  • Pay a $10 copay for routine comprehensive eye exam, limited to one per calendar year.
  • Pay a $25 copay for eyeglass frames and lenses (single vision, bifocal, or trifocal lenses).
  • Use allowance to help cover frames ($150 per year) or contact lenses ($150 per year).
  • Receive discounts for other vision materials.

Keep in mind, it may cost you more if you receive services from an out-of-network provider. Note: You don't have to enroll for MDLZ-sponsored medical coverage to elect vision coverage.
