The Mondelēz Global Medical Plan (Medical Plan) provides comprehensive coverage for certain services and procedures, including preventive care and prescription drugs. Whether coverage through MDLZ is available to you depends on your union's negotiated collective bargaining agreement (CBA).
If eligible, see your collective bargaining unit’s Health Care Benefits Supplement. It’s posted in the Reference Center on MyBenefits Online and includes which carrier—Blue Cross and Blue Shield of Illinois (BCBS) or Kaiser Foundation Health Plan of the Northwest (Kaiser NW)—administers your coverage. Depending on the carrier, here’s what’s included with medical coverage:

- Vision coverage through EyeMed or the Blue 365 vision discount program (see the Vision page for more information).
