The Mondelēz Global Medical Plan (Medical Plan) provides comprehensive coverage for certain services and procedures, including preventive care and prescription drugs. Whether coverage through MDLZ is available to you depends on your union's negotiated collective bargaining agreement (CBA).

If eligible, see your collective bargaining unit’s Health Care Benefits Supplement. It’s posted in the Reference Center on MyBenefits Online and includes which carrier—Blue Cross and Blue Shield of Illinois (BCBS) or Kaiser Foundation Health Plan of the Northwest (Kaiser NW)—administers your coverage. Depending on the carrier, here’s what’s included with medical coverage:

Medical coverage through BCBS: Includes:
- Prescription drug coverage through CVS Caremark (see the Prescription Drugs page for more information).
- Vision coverage through EyeMed or the Blue 365 vision discount program (see the Vision page for more information).
- Access to Vida Health’s and Hinge Health’s services to support your unique care needs (see the Specialty Medical Solutions page for more information).
Medical coverage through Kaiser NW: Includes:
- Prescription drug coverage through Kaiser NW (see MyBenefits Online for more information).
- Vision coverage through EyeMed (see the Vision page for more information).

You can always log in to your MyBenefits Online account to see your current coverage elections, coverage cost (if any), and for additional coverage and carrier-related details.

ABOUT YOUR CARRIER’S NETWORK

Your carrier has its own network of providers. And, when you stay in-network, covered services are provided at a discount.

To confirm whether your doctor, hospital, or other care provider participates in a carrier’s network, be sure to use the provider look-up tool on MyBenefits Online. It’s also a good idea to double-check directly with your provider to make sure they participate in the carrier’s network where you live. Your provider will have the most up-to-date information about their participation.

TELEMEDICINE

With telemedicine, you can talk to a doctor 24/7. Your carrier offers telemedicine for a variety of services (e.g., general medical, behavioral/mental health). Per-visit costs vary by carrier and type of service received; but are often less than what you would pay for in-office care.

SPOUSAL SURCHARGE & CARRIER PROCESSES

A spousal surcharge may apply

Depending on your union’s negotiated CBA, a surcharge may apply if you decide to cover your spouse and they have their own medical coverage available elsewhere. See your collective bargaining unit’s Health Care Benefits Supplement for the additional monthly surcharge that may apply for your spouse’s medical coverage. You can also view your cost (if any) when you log in to your account on MyBenefits Online.

WHO DO YOU NEED TO COVER?

As long as your union’s negotiated CBA provides for medical coverage for you and your family members, you can cover your dependents. Eligible dependents include your:

Legal spouse
Same- or opposite-sex domestic partner
Child(ren)

Consider whether it makes sense for you and your family to be covered under the Medical Plan or through your spouse’s/domestic partner’s employer (if applicable).
