COVERAGE FOR A BROAD RANGE OF DENTAL SERVICES
The Mondelēz Global Dental Plan (Dental Plan) covers a broad range of dental services, including preventive care. The coverage that’s available to you depends on your union’s negotiated collective bargaining agreement (CBA).
If eligible, see your collective bargaining unit’s Health Care Benefits Supplement. It’s posted to the Reference Center in your MyBenefits Online account and includes which carrier—Delta Dental of Illinois (Delta Dental) or Kaiser Foundation Health Plan of the Northwest (Kaiser NW)—administers your coverage. The Reference Center also includes your Summary Plan Description (SPD), which details what’s included in your dental coverage and how the Dental Plan pays benefits.
You don’t have to enroll for medical coverage through MDLZ to elect dental coverage. And, you can always log in to your MyBenefits Online account to see your current coverage elections, your coverage cost (if any), and additional coverage and carrier-related details.
Looking to Save money with a Preferred Provider?
Network providers offer services at a discount, plus conveniently submit claims on your behalf. If you wish, you can use out-of-network providers. But, your out-of-pocket costs may be higher. In addition, you may be responsible for filing your own claims.
