Do I need an ID card?

If your union’s negotiated collective bargaining agreement (CBA) determines you’re eligible to participate in the Mondelēz Global Medical Plan (Medical Plan) and you enroll for medical coverage, you should receive an ID card from your carrier. Contact your medical carrier directly if you need a replacement. (Note: Your carrier may provide ID cards electronically via their website and/or app.)
