Eligibility Rules
MDLZ offers health and welfare coverages to Those Who Meet the Eligibility Requirements*
If your union's negotiated collective bargaining agreement (CBA) allows, you may enroll the following eligible dependents for certain coverages (i.e., medical, dental, and/or vision coverage):
- Your legal spouse
- Your same- or opposite-sex domestic partner
- Your children
NOTE: If your domestic partner or domestic partner’s child isn't your tax dependent, the full cost of their medical and dental coverage (if applicable) is taxable income to you. This amount is “imputed income” and will be included in the income amount shown on your pay statement and W-2 form. Applicable taxes on imputed income will be withheld each pay period.
* Eligibility for coverages, including certain optional and voluntary coverages and programs, differs across union-represented employees and is dependent upon each union’s negotiated CBA. Refer to your union’s CBA, your collective bargaining unit’s Health Care Benefits Supplement, as well as “The Mondelēz Global LLC Group Benefits Plan Wrap Summary Plan Description (Wrap SPD)” for details regarding eligibility requirements. You can find your Health Care Benefits Supplement and Wrap SPD in the Reference Center on MyBenefits Online. For a copy of your union's CBA, contact your union representative.
